To submit your photograph for inclusion in the exhibition
catalogue, to be published with Blurb Books, please send a digital file of your photo (see sizing instructions below) and
cut and paste this form into a Word document or send the information in an e-mail to photos@vtphotoworkplace.com

Permission to reproduce work
I would like my photograph
to be included in the exhibition catalogue, POETIC OBJECTS: STILL LIFE AS SUBJECT, published with Blurb books in
conjunction with a juried exhibition at the PhotoPlace Gallery. Only the artist’s
name and the title of the work will appear in the caption. The book will be available for preview and purchase directly from Blurb Books.

Please provide jpg files for this purpose with these specs: 300 dpi at 100% scale, 2067 pixels on longest side, sRGB color. Even if your file is a black and white photo, save it as RGB, not grayscale. All files will be permanently deleted from the work folder immediately after
the book has been designed.
I hereby grant one-time fine arts use for the reproduction of my photograph
for this sole purpose. I certify that I have permission to reproduce the likeness of recognizable individuals
and can provide a model release upon request.
